We’re looking for a minimum data set (MDS) nurse who can effectively communicate with the healthcare team, patients, and families to ensure the delivery of high-quality care. - Assessment and Data Collection: Conduct and oversee the completion of the MDS, a federally mandated clinical assessment of all residents in Medicare or Medicaid certified nursing homes.
- Care Planning: Use MDS assessments to create individualized care plans for patients, ensuring their health, emotional, and psychosocial needs are met.
- Collaboration: Work with other healthcare professionals to gather patient data and ensure quality patient care.
- Compliance: Ensure compliance with all federal and state regulations regarding the MDS and patient care.
- Training: Train and educate nursing staff on MDS procedures and updates.
- Quality Assurance: Conduct quality assurance audits and review records to ensure accuracy and completeness.
- Education: Must be a licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) with experience in long-term care settings.
- Experience: Prefer at least 1 year of MDS experience. Familiarity with electronic medical record (EMR) data and PCC MDS software is essential.
- Skills: Strong communication, organizational, and leadership skills are crucial.
