Requirements
Education:
- M.D. or D.O. Required
- Board Certified in Family Medicine or must obtain Board Certification within 3 years of residency completion: Required
Certifications & Licensure:
- Current D.E.A. Certification
- Ability to obtain full, unrestricted, Wisconsin Medical License
- Basic Life Support (BLS); Advanced Cardiac Life Support (ACLS); Advanced Life Support Obstetrics (ALSO); Neonatal Resuscitation Program (NRP) - Required (will provide training onsite)
Minimum Formal Training:
Successful completion of an approved residency training program in family medicine. Board certification or board eligible for certification in family medicine.
Applicant must provide documentation of 3-4 months obstetrical rotation during family practice residency, with 50 patients delivered. If applicant has been out of residency for 2 years or more, delivery logs for the last 2 years will be required.
Ability and willingness to perform C-Sections is strongly desired.
Required Previous Experience:
Applicant must be able to demonstrate that he or she is currently engaged in an active medical practice in family medicine; or that he or she has recently completed a residency in family medicine. Those desiring to hold family medicine privileges must also be able to demonstrate their practice includes the provision of care to family medicine patients; and those recently completing an approved residency program must be able to demonstrate that appropriate training was included in the residency program.
