Description
- Provide patient-centered, comprehensive primary care services consistent with the patient centered medical home care delivery model.
- Participate in the selection of suitable treatment plans for all patients.
- Assist other care providers and extended care teams to comply with the Health Center’s patient care practices, policies, and procedures.
- Participate in the selection of cost-effective treatment modalities.
- Provide continuous availability for medical and technical questions to the employees, patient care staff, and ancillary care providers including after hours coverage.
- Collaborate with the management team as needed to ensure improvement of patient care processes, policies, and procedures.
- Cooperate with clinical outcome reviews and measurements standards as implemented.
- Other duties as assigned.
- The Nurse Practitioner is required to be a board-certified, Washington-licensed NP with a minimum of 5-7 years of professional clinical experience in primary care practice. Strong preference for experience in an integrated, multi-disciplinary medical home model.
- Proficient in promoting the Health Center and comfortable in public speaking and education settings including the development of topical presentations.
- Current BLS (Basic Life Support) certification.
- Physical ability to execute job responsibilities that may include: Standing or stooping for long periods of time, Mild physical labor to complete the tasks at hand, Exposure to blood, chemicals, and other bodily fluids.
Salary : $75 - $85
