Position Summary
The Resident Care Coordinator individual works closely with the Executive Director, Resident Services Director to make sure all residents receive the proper care and attention in a dignified, trusting environment while being sure to record and monitor the progress of each resident. This individual also works closely with doctors and other third-party health care providers to make sure all residents are appropriately cared for.
Duties and Responsibilities
● Answer incoming phone calls and take appropriate action for each call.
● Participates in on-call rotation.
● Keeps the staff schedule up to date and posted at least one week in advance.
● Covers as PCP or QMAP when short staffed.
● Participate in monthly Hospice and PACE Interdisciplinary Team meetings.
● Responsible for communication with pharmacy and doctor’s offices with regards to medication orders and refills.
● Reports resident care concerns to the Executive Director, Resident Services Director, and Memory Care Director.
● Monitor’s medication carts for cleanliness and condition and pulls expired medications.
● Answers staff, family and resident questions about medications concerns.
● Ensures progress notes are documented for all residents regarding medication concerns and care.
● Works closely with the Pharmacy to assure accuracy in medication orders ( Foursquare, PACE, VA, Hospice, and local pharmacies.)
● Ensures all residents have a minimum of seven days’ worth of medications at all times.
● Ensure all orders are signed and uploaded prior to activating.
● Responsible for making sure all discontinued or changed medications are removed from the EMAR before new medications are activated.
● Participates in Monthly falls management meetings.
● Completes medication triple checks for all resident medications.
● Completes monthly medication disposal for expired and discontinued medications.
● Performs weekly, monthly, and quarterly medication audits. Including all backstock medications.
● Perform bi-weekly resident services supply audits ( gloves, medication cups, water cups, spoons, pill crusher pouches, etc.)
● Demonstrates respect and regard for the dignity of all visitors and fellow employees to ensure a professional, responsible, and courteous
environment.
● Answer’s emergency and routine calls via the pendant/ call light system.
● Other tasks as assigned.
Qualifications
● High school diploma or equivalent
● Must be QMAP certified
● Must have a minimum of 6 months experience
● Basic computer knowledge
● First aid and CPR certified
● Experience in a senior living community preferred
● Passionate about working in an elderly community with exceptional customer service
● Ability to work independently, effectively manage own time and multi-task in a fast-paced environment
● Effectively communicate with staff, third-party healthcare providers and elderly residents
● Must pass pre-employment drug screening, criminal background investigation, and reference inquires
Physical Requirements
● Consistently required to stand, walk, bend, squat, reach, grasp, twist and turn, talk, listen and hear.
● Medium physical strength required to perform work
