Job Summary: The Art and Wellness Care Specialist is responsible for providing comprehensive behavioral health treatment services to Crossroads clients in an outpatient setting. Under the supervision of the Outpatient Integrated Care Manager, the Art and Wellness Care Manager coordinates and monitors the assessment, treatment, and facilitates healing art groups with Art Of The Soul© / substance abuse groups, and the planning process and delivery of services outlined in outpatient client treatment plans.
Supervisory Responsibilities:
· None
Essential Functions:
· Manages intake and discharge planning of Outpatient clients.
· Coordinates care with clients, Probation and Parole Officers and Federal courts.
· Ensures that all outpatient participants complete the individual service plan for contracted behavioral health services.
· Facilitates Outpatient counseling behavioral health groups (including psychoeducation and Art Of The Soul© groups in person and remotely.)
· Attend mandatory in-person department staffing’s, clinical oversight and Adult Recovery Treatment (ART) meetings.
· Assist with risk screenings, identify needs and complete client enrollment process.
· Provides individual behavioral health interventions and case management services.
· Maintains caseload with accuracy as set by manager.
· Meet individual monthly billable services goal.
· Maintains Art Of The Soul© monthly supplies.
· Maintains art studio to the standards of the franchise.
· Promote community involvement for client programming participation.
· Perform other duties, as required.
Qualifications:
· Must meet Arizona Department of Health Services personnel member requirements of: at least 21 years old; or licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice.
- Art Of The Soul© training completed within 90 days of hire.
· Current CPR/First Aid/Narcan training.
· Current and annual negative TB Test.
· Current and valid Fingerprint Clearance Card.
· Clean Motor Vehicle Record (MVR) check performed annually and upon hire, preferred.
· Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check.
Knowledge, Skills and Abilities:
· Knowledge of addiction recovery processes and the continuum of care for persons with substance use disorder.
· Must be proficient with computer, Microsoft Office and EMR systems.
· Demonstrated strong documentation and organization skills.
· Ability to communicate effectively at multiple levels, verbal and written.
· Ability to work independently and as a member of a team.
Education & Experience:
· Minimum of High School Diploma/GED, required.
· 1 year related experience, and/or training or equivalent combination of education and experience.
· Experience in behavioral health setting with working knowledge of case management services and planning delivery.
Work Environment:
· Fast paced behavioral health residential and outpatient setting.
· Primarily indoors, however, some outdoor work may be requested.
Physical Demands:
· While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds and seldom lift and/or move >100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.
Shift Requirements:
· Days, nights, weekends and holidays.
Travel:
· Valid AZ Driver’s License.
· May include travel between sites and other locations.
· Local and state wide travel when requested or required.
