Company Summary: Crossroads is an Arizona Department of Health Services licensed substance abuse treatment provider with proven expertise in serving men, women, and Veterans with Substance Use Disorder through the provision of high quality, successful, and affordable residential and outpatient recovery programs.
Due to federal contracts the following items are prohibited: Medical/Recreational marijuana, current parole, or probation.
Job Summary: The Family Nurse Practitioner is responsible for providing direct primary care needs (Acute and Chronic Care) of adult clients. The provider will assess the physical needs of the client and implement a plan of care through diagnostic, preventative, and therapeutic services in accordance with all state, federal, and licensing standards and guidelines.

Essential Functions:
- Provide direct care to Crossroads clients, focusing on health promotion and treatment of acute and chronic illness needs.
- Identify, develop, and implement client education programs.
- Orient the clients to available services within Crossroads.
- Act as an advocate in directing clients to available clinic and community resources.
- Order appropriate cost-effective diagnostic tests.
- Interpret findings to form diagnoses.
- Negotiate appropriate treatment plans with clients.
- Prescribe medications as allowed by scope of practice.
- Educate clients regarding condition, treatment, and possible outcomes.
- Perform complete and focused physical exams as indicated.
- Provide health care services for identified prevention and health promotion needs.
- Collaborate with other members of the staff and volunteers.
- Make appropriate referrals.
- Evaluate and modify treatment as indicated by client outcomes.
- Other duties as assigned.
- Maintain accurate medical records using EMR and proper forms in a timely manner.
- Oversees and trains clinical staff and volunteers on data entry into the electronic health records system and provide oversight that original data is entered correctly.
- Supervises and precepts student volunteers.
- Consistently use supplies and equipment in a cost-effective manner.
- Recommend needed supplies and equipment for delivery of care.
- Participate in quality improvement activities as directed by supervisor.
- Provide education to clinic staff as needed.
- Communicate effectively with clients and their families in a respectful manner.
- Maintain an acceptable caseload to meet productivity needs.
- Utilize updated written protocols as mandated by the Arizona Board of Nursing Statutes and Regulations.
- Ensure that care provided is consistent with community standards of care.
- Follow established safety precautions in performance of duties to ensure a safe environment.
- Support policies and philosophy in delivering health care as set forth by Crossroads.
- Performs other duties as needed.
Qualifications:
- Current State of Arizona RN license.
- Current license, in good standing, to practice as a Nurse Practitioner and state authority for advanced practice, including possession of prescription and dispensing privileges required.
- Certified by the Specialties Board in his/her respective specialty.
- Valid DEA license.
- Current CPR/First Aid/Narcan training, BLS required.
- Current and annual negative TB Test.
- Current and valid Fingerprint Clearance Card.
- Clean Motor Vehicle Record (MVR) check performed annually and upon hire, preferred.
- Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check.
Knowledge, Skills, and Abilities:
- Ability to adhere to all in-house and legal regulations.
- Familiarity with EMR records systems.
- Strong leadership, communication, problem solving and organizational skills.
- Ability to work independently and as a member of a team.
Education & Experience:
- Master’s degree in nursing required, DNP preferred.
- Experience with clinical risk management and safety protocols.
Work Environment:
- Fast paced behavioral health clinic setting.
- Primarily indoors, however, some outdoor work may be requested.
Physical Demands:
- While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds and seldom lift and/or move >100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.

- Valid AZ Driver’s License.
- May include travel between sites and other locations.
- Local and statewide travel when requested or required.
