Job Summary: The Psychiatric Mental Health Nurse Practitioner is responsible for providing direct psychiatric care needs (Acute and Chronic Care) to individuals of the Crossroads Community Care Clinic. Assessment of the psychiatric needs of the client and implementation of a plan of care through diagnostic, preventative, and therapeutic services in accordance with all state, federal, and licensing standards and guidelines.

Essential Functions:
· Diagnoses psychiatric disorders and mental health conditions. Distinguishes between physiologically and psychologically based disorders and makes appropriate diagnoses.
· Assesses patients’ mental and physical status based on the presenting symptoms and complaints.
· Refers to individual, group, and family psychotherapy for those with chronic or acute mental disorders when appropriate.
· Documents patients’ medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, and outcomes.
· Prescribes psychotropic medications as allowed by state regulations and collaborative practice agreements. Monitors patients’ medication usage and response.
· Evaluates patients’ behavior to formulate diagnoses or assess treatments.
· Educates patients and family members about mental health and medical conditions, preventive health measures, medications, and treatment plans.
· Collaborates with interdisciplinary team members including psychiatrists, psychologists, and nursing staff, to develop, implement, and evaluate treatment plans.
· Strong skills to manage, coordinate, and evaluate the care and education of the patient and family along the health care process.
· Ability to manage patients who need to be closely monitored.
· Collaborate with other members of the staff and volunteers.
· Follow Crossroads protocols for medications and therapies.
· Maintain accurate medical records using EMR and proper forms in a timely manner.
· Oversees and train clinical staff and volunteers on data entry into the electronic health records system and provide oversight that original data is entered correctly.
· Track client data that will demonstrate measurable success of Clinic services. Generate and analyze reports to document outcome measures from the electronic records. Must be proficient in developing & analyzing outcome measures.
· Supervise and precept student volunteers.
· Recommend needed supplies and equipment for delivery of care.
· Participate in quality improvement activities as directed by supervisor.
· Provide education to clinic staff as needed.
· Communicate effectively with clients and their families in a respectful manner.
· Maintain an acceptable caseload to meet productivity needs.
· Utilize updated written protocols as mandated by the Arizona Board of Nursing Statutes and Regulations.
· Ensure that care provided is consistent with community standards of care.
· Follow established safety precautions in performance of duties to ensure a safe environment.
· Support policies and philosophy in delivering health care as set forth by Crossroads.
· Performs other duties, as required.
Qualifications:
· Current State of Arizona RN license.
· Current license, in good standing, to practice as a Nurse Practitioner and state authority for advanced practice, including possession of prescription and dispensing privileges required.
· Valid DEA license.
· Certified by the Specialties Board in his/her respective specialty.
· Current CPR/First Aid/Narcan training, BLS preferred.
· Current and annual negative TB Test.
· Current and valid Fingerprint Clearance Card.
· Clean Motor Vehicle Record (MVR) check performed annually and upon hire, preferred.
· Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check.
Knowledge, Skills, and Abilities:
· Ability to adhere to all in-house and legal regulations.
· Familiarity with EMR records systems.
· Strong leadership, communication, problem solving and organizational skills.
· Ability to work independently and as a member of a team.
· Ability to communicate effectively at multiple levels, both verbal and written.
Education & Experience:
· Master’s degree in Nursing required, DNP preferred.
· 2 years’ clinical experience in a clinical care setting, preferred.
· Experience with clinical risk management and safety protocols.
Work Environment:
· Fast paced behavioral health clinic setting.
· Primarily indoors, however, some outdoor work may be requested.
Physical Demands:
· While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds and seldom lift and/or move >100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.

· Valid AZ Driver’s License.
· May include travel between sites and other locations.
