Company Summary: Crossroads is an Arizona Department of Health Services licensed substance abuse treatment provider with proven expertise in serving men, women, and Veterans with Substance Use Disorder through the provision of high quality, successful, and affordable residential and outpatient recovery programs.
Due to federal contracts the following items are prohibited: Medical/Recreational marijuana, current parole, or probation.
Job Summary: The Risk Stratification Specialist is responsible for assessing risk levels of each patient that admits into the Crossroads residential program, and for ensuring the proper treatment and services are provided to each client.
Supervisory Responsibilities:
· None
Essential Functions:
· Responsible for screening and determining each client’s risk factors.
· Providing direct care and assessing client needs one on one in person.
· Verify client’s insurance and work with in AHCCCS guidelines.
· Making necessary determinations in each client’s care.
· Responsible for daily coordination of care with clinic providers as well as admissions.
· Assisting members by increasing employment supports, reducing hospitalizations, reducing recidivism, increasing daily functioning as well as assisting with ongoing mental health and physical stabilization.
· Document encounters in multiple EHR systems.
· Responsible for communication with the scheduler as well as facilities.
· Responsible for having clients placed on the correct CCOM team and scheduling intakes and for the CCOM teams to help with all other needs.
· Coordinate with 3rd party organizations and agencies, such as pharmacies, detoxes and treatment centers.
· Perform other duties as necessary.
Qualifications:
· Must meet Arizona Department of Health Services personnel member requirements of: at least 21 years old; or licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice.
· Current CPR/First Aid/Narcan training.
· Current and annual negative TB Test.
· Current and valid Fingerprint Clearance Card.
· Clean Motor Vehicle Record (MVR) check performed annually and upon hire, preferred.
· Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check.
Knowledge, Skills, and Abilities:
· A working knowledge of addiction, recovery, and twelve step programs.
· Must be proficient with computer, Microsoft Office and EMR systems.
· Well-developed written and verbal communication skills and ability to interact effectively with internal and external audiences.
· Must maintain high ethical standards and demonstrate sound judgment, problem solving skills, and strong organizational skills.
· Ability to communicate effectively at multiple levels, both verbal and written.
· Ability to work independently and as a member of a team.
Education & Experience:
· Minimum of High School Diploma or GED required.
· Preferred - 1 year experience providing direct patient care and working in multiple settings related to substance abuse and mental health.
· Preferred - 1 year or more of experience working in a mobile outpatient case management team.
· Peer Recovery Support Specialist certification from an Arizona approved agency or obtained within 90 days of hire, required.
Work Environment:
· Fast paced behavioral health setting.
· Primarily indoors, however, some outdoor work may be requested.
Physical Demands:
· While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds and seldom lift and/or move >100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.
Shift Requirements:
· Days, nights, weekends, and holidays.
Travel:
· Valid AZ Driver’s License.
· May include travel between sites and other locations.
· Local and statewide travel when requested or required.
