What are the responsibilities and job description for the LMSW/LPC - (PRN position) position at CrossRoads Intensive Outpatient Clinic?
Educational requirements:
Licensed master’s in social work (LMSW)--- masters degree in social work
Licensed clinical social worker (LCSW)---- doctoral degree in social work
Licensed professional counselor (LPC) --- masters in counseling

CrossRoads IOP provides therapy services, discharge planning, and treatment planning topatients admitted to the program by a Licensed Social Worker, Licensed Therapist or Counselor. Therapy sessions may be conducted by qualified staff such as: Licensed Clinical Social Workers, Licensed Master’s Social Worker, Licensed Marriage and Family Therapist, or Licensed Professional Counselor, under the general supervision of the psychiatrist or APN.
PROCEDURE:
1. The social worker or therapist will complete a comprehensive Psychosocial History/Assessment within three days.
2. The social worker or therapist is responsible for attending and assisting psychiatrist/APN in facilitating Interdisciplinary Treatment Team Meetings.
3. The social worker or therapist will make recommendations and address family and community resource issues in treatment team meetings.
4. The social worker or therapist will be responsible for ensuring that discharge planning is started at the time of admission and documented in the Interdisciplinary Progress Note. Updates to Discharge Planning will be made throughout the treatment process as indicated and documented in the Interdisciplinary Progress Notes. (See Discharge Planning policy 205.1)
5. The social worker or therapist will be actively involved in the direct care of the patient, providing individual, group, family therapy and teaching Psychotherapy session, if needed. Each intervention will be documented on the day of treatment according to Medical Record Documentation Policy (Policy 220.0)
6. The social worker or therapist, under direction of Psychiatrist/APR, is responsible for the clinical decisions involving the patient’s psychiatric illness.
7. The social worker or therapist may be responsible for contacting outside referral sources to inform them of the referred patient’s admission, progress and discharge, with consent of patient.
8. The social worker or therapist will be responsible for documenting a Master Treatment Plan, Treatment Plan Review as indicated.
Therapist duties:
- Conduct 3 group sessions daily, individual and telehealth sessions when needed
- Plan topics for group sessions
- Document patients therapy notes daily
- Document and update monthly treatment plans
- Complete a psychosocial assessment on new patients
- Complete a master treatment plan on new patients
- Make recommendations and address family & community issues that arise
- Maintain open communication between members of the treatment team (physician, nurse, other therapist, CNA/transporters)
