GENERAL DESCRIPTION

This position is responsible for protecting the health, safety, and well-being of the residents and providing clinical and technical support whether that resident gets foster/adoptive parents or has permanent residency in the facility. The main objective for this position is to effectively work towards achieving permanency for children while actively assessing the needs of the child. Case Managers work directly with their clients. They review their records, speak with their family members, and understand their history to assess their needs and conditions properly. They partner with organizations on behalf of their residents to find the best options.
YOUTH RESIDENTIAL CARE COORDINATOR RESPONSIBILITIES
- Work with case team to ensure the achievement of quality care and outcomes for the youth
- Adhere to professional standards as outlined by protocols, rules and regulations
- Facilitate multiple care aspects (case coordination, information sharing, etc.)
- Involvement daily in the house program and interact with residents to keep track of their progress and to ensure satisfaction for all parties involved.
- Track and record case information, complete forms, and produce reports. Manage the cost of care wisely, and provide timely and accurate information to the supervisor.
- Drive change by identifying areas where performance improvement is needed
- Manage case assignments, develop service plans, track progress, and execute case closures
- Advocate for clients with advice and information about treatment options and clinical status, evaluating medical necessity and appropriating resources
- Facilitate care and treatment via scheduling, information sharing, and case coordination
- connecting clients with professional services and maintaining consistent communication with clients to assess their wellbeing and make changes to their treatment plan as needed.
- Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered
- Meet with the in house management team twice a week to report out and collaborate.
QUALIFICATIONS
Must have a minimum of 3 years of experience working with adolescents and/or their families, especially behaviorally challenged and adjudicated youth. Previous experience supervising staff, and experience developing programs is helpful. Must have previous experience leading teams. Extensive familiarity with youth care principles, understanding of guidelines for social work. Must have a bachelor degree in social work, counseling, child development, psychology or a related field. Must have valid driver's license, acceptable driving record per agency liability insurance, and sufficient auto insurance (minimum of "100/300" coverage). No convictions or license revocations that would fail a caregiver background check.
