We are currently searching for a dynamic individual to become the CARE FACILITATOR for our unit. This position provides case management services to patients at the residential units.
All applicants must undergo pre-employment drug screening,
which includes testing for marijuana and background checks.
ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
- Responsible for developing case management planning, assessments, and implementation.
- Acts as liaison, establishing networks and collaboration with hospitals, community, state, and human service agencies about client services and needs.
- Provides monitoring of U.A. sample collection and monitors the medication requirements of proper charting and reporting.
- Assists counselors with client intake, transfer, and discharge assessments.
- Coordinates and maintains client basic needs (i.e., clothing, medical, judicial, religious, etc.) from available resources.
Qualifications
QUALIFICATIONS:
Education:
High School diploma or GED required. AA or BA in a human service-related field is preferred.
Experience:
Minimum of six (6) months experience working in a substance abuse or similar field.
Previous case management experience preferred.
Other:
Must obtain CAT certification within 18 months of their hiring date.
Must understand substance abuse issues and dependency intervention and treatment delivery systems.
Basic computer skills required.
CPR and first-aid certification. (The agency pays for initial training and renewals).
Must be able to take vital signs and recognize medical problems.
Valid driver's license and insurability.
