Crossroads4Hope, A Network of Cancer Support (CR4H), has been improving the lives of people with cancer and their loved ones for more than two decades. We take on the cancer journey with people diagnosed, caregivers, and loved ones, providing a network of support, information, resources, and professionals who fill critical gaps in care that often occur between medical appointments and within the healthcare system itself. Our services, which are free of charge, are designed to address the emotional, psychological, social, and practical challenges and gaps in care that come with a cancer diagnosis. Our mission is to transform the cancer experience by reducing the burden on all those who are impacted, especially in low-income and marginalized communities. Led by social work principles, most notably social justice, we are experts in patient activation and delivering psychosocial interventions. We move individuals through the health crisis caused by cancer to become activated with the behaviors that enable them to take control of their health and care through access to resources and programs of support, education, well-being, and hope.
The Director of Oncology Social Work will be based in New Jersey and lead our community-based cancer support initiatives nationally. This role will oversee the delivery of clinical services, including individual, group, and family interventions that address psychosocial and practical needs, address health disparities, and integrate cultural competence and humility into person-centered supportive nonmedical care. This role will lead the clinical operations across the organization, including the management of intakes and assessments, case management, and member engagement across the cancer continuum. The Director will also play a critical role in integrating and expanding our MyGo2Support platform, which offers remote telehealth services and responds to patients' distress, depression risks, and social needs.
Key Responsibilities
Program Development & Management:
- Lead the creation, implementation, and evaluation of community-based oncology social work programs that ensure various interventions are available to diverse populations across the cancer continuum.
- Enhance and optimize the MyGo2Support direct-to-mobile platform, ensuring enrollment, timely response to escalations, and tailored support for social and emotional needs using evidence-based screening tools.
- Collaborate with the Public Health and Education teams to ensure programming aligns with community needs assessments and stays current with industry trends.
Clinical Program Delivery:
- Provide direct psychosocial support and clinical interventions for complex casework in person and virtually. These include treatment decision support, resource navigation, referral, financial assistance, triage, and response to escalations in MyGo2Support.
- Safeguard a timely and effective response to all referrals (self/provider), including MyGo2Support, through clinical triage and follow-up.
- Establish and manage standard operating procedures for clinical documentation in our Salesforce CRM.
Manage processes for onboarding new members and facilitate transitions into
Crossroads4Hope's wraparound support, education and nutrition workshops, and mind-body classes.
- Develop strategies to monitor the engagement and retention of Crossroads4Hope members through outreach, regular check-ins, and personalized care plans and ensure continuity of care.
- Ensure processes are in place for transitioning members across the cancer continuum, including appropriate safeguarding for patients and caregivers transitioning out of active programs.
- Use data-driven approaches to continuously refine these processes, improving the overall member experience and outcomes.
Clinical Supervision & Staff Development:
- Manage, supervise, and oversee all appropriate clinical staff, including oncology social workers, child life specialists, and contracted mental health professionals.
- Mentor and guide staff in culturally competent, trauma-informed care, ensuring that team members are equipped to handle complexities in the psychosocial landscape.
- Ensure that all services meet the highest standards of ethical and clinical practice.
Partnership Development, Collaboration & Teamwork:
- Develop clinical partnerships to increase referral pathways from oncology practices, institutions, and community providers and to enhance care coordination.
- Collaborate with our Public Health Outreach and Education teams to develop and implement community interventions in underserved areas.
- Foster interdisciplinary collaboration to ensure holistic, seamless care for patients and families.
Requirements:
- Master of Social Work (MSW)
- Must hold an active Licensed Clinical Social Worker (LCSW) credential.
- Oncology Social Work Certification (OSW-C), required or pending certification
- At least 5-7 years of experience in oncology social work, with demonstrated leadership in a clinical or community-based setting.
- Proven expertise in overseeing onboarding, intake, case management, and providing clinical supervision.
- Commitment to cultural humility, trauma-informed care, and health equity in all practices.
- Experience with evidence-based mental health screeners, such as the PHQ-2, PHQ-9, and Health Leads Social Needs, is required.
- Experience with telehealth is a plus.
- Bilingual in Spanish (preferred)
