The Nurse Proxy position provides quality nursing services to agency clients and their families, in support of clients maintaining compliance with prescribed medications, learning, and applying self-care tools for better health, and preventing or reducing hospitalizations.
Essential Functions

- Meets with and interviews clients prior to medication clinic appointments to gather current health information.
- Verifies current treatment plans and prepares Medication Management progress notes for Med Providers.
- Provides direct nursing services to child and adult clients under the direction of the Assistant Director and/or Medical Director, in an effort to maintain medication compliance and to promote stable mental and physical health.
- Processes client inquiries and requests for prescription refills by collaborating with area pharmacy staff and primary care providers to assist clients in obtaining psychotropic medications as prescribed by medication clinic providers. Completes documentation for the medical record and client files.
- Maintains files of client prescription copies written by medication clinic providers to enable easy access to prescription history and information. Forwards laboratory reports to appropriate parties and records receipt and distribution on a dedicated log.
- Assists Med Clinic Customer Service and Assistant Director in the triage of referrals to medication clinic and coordinates the scheduling of appointments to promote coordination of care among other services provided by CrossWinds, hospitals, schools, and others.
- Administers psychotropic medication injections to clients as prescribed and completes the progress note for the medical record.
- Complies with appropriate policies and procedures for safe medication practices and secure custody and storage of all medications. Reinforces teaching lessons with clients on an individual basis by discussing health issues such as healthy eating habits and checking client’s blood pressure.
- Works in partnership with patient assistance programs to obtain financial assistance for low-income individuals to obtain psychotropic medications as prescribed by CrossWinds prescribing providers and/or to obtain financial assistance for low-income clients.
- Partners with other healthcare providers to identify other medical conditions and appropriate inventions to promote health and wellness by observing and coordinating treatment of acute and chronic medical conditions.
- Assists in completion of discharge summaries upon direction from Medical Records for clients who receive medication clinic services only. Obtains the prescriber’s signature on discharge summary.
- Maintains accurate and complete records of all client services, as defined by state licensing agencies, funding agencies, and CrossWinds standards and requirements. Submits progress notes, treatment plans, timesheets, and all work-related paperwork in a timely manner and in acceptable format as determined by agency requirements. Establishes and maintains a positive working relationship and professional demeanor with clients, external community partners, CrossWinds staff members, and visitors. Attends professional workshops and seminars in order to maintain and improve professional skills and meet licensure/certification requirements.
Work Environment
This job operates in the community, client homes, and a professional office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.

Education & Experience |
Graduate of an accredited school of practical nursing or associate degree in Nursing is required. |
Certifications & Licenses |
Licensure as a Licensed Practical Nurse from the Kansas State Board of Nursing is required. |
Computer Skills |
Computer experience in Microsoft Office programs and basic computer fluency. |
Additional Qualifications |
Ability to work with persons with serious emotional disturbance and/or severe and persistent mental illness; possess demonstrated nursing practice and interpersonal skills and ability to utilize sound nursing judgment and react effectively in a variety of human service situations; ability to work independently and as a member of a therapeutic team. |
