Crouse’s Addiction Treatment team at Erie Boulevard provides a comprehensive system of assessment and outpatient treatment services for individuals with substance use disorders by integrating physical, mental and spiritual healthcare practices for patients and families. The Addiction Treatment team is hiring an Addiction Treatment Case Manager to work Monday- Friday 7:30am-4:00pm.
Pay Range: $24.00-$29.00/hour based on verified education and experience
Addiction Treatment Case Manager Position Responsibilities:
- Addresses social determinants of health that compromise an individual’s ability to successfully engage in addiction treatment, e.g.
- Transportation
- Housing
- Access to healthcare
- Food insecurity
- Collaborate with the patient’s Multi-Disciplinary Team to develop and manage a comprehensive recovery plan.
- Build and maintain professional relationships with community organizations.
- Facilitate referrals for needed services.
- Obtain insurance authorization.
- Complete all required documentation and billing in accordance with program standards.
Addiction Treatment Case Manager Position Requirements:
- Associate's Degree in Human Services or a related field is required.
- 2 years of experience in Addiction Treatment or Behavioral Health.
- Years of experience can be substituted with a Bachelor’s or Master’s degree in related field.
- CASAC designation encouraged.
- Strong organizational and communication skills.
- Knowledge of local resources.
- Experience with Microsoft Office including Outlook and Excel.
