Position Overview:

As an LPN at Troy Rehabilitation and Healthcare Center, you will be responsible for providing direct patient care, supporting residents' physical and emotional well-being, and working collaboratively with our healthcare team. You will use your clinical expertise to administer medications, monitor patient progress, and ensure our residents receive the highest quality care.

Key Responsibilities:

• Administer medications, treatments, and procedures as prescribed by physicians.
• Monitor and assess residents' health, responding to changes in condition and ensuring proper care.
• Assist in developing and updating care plans to meet the individual needs of each resident.
• Provide leadership and supervision to nursing assistants and other support staff.
• Ensure accurate documentation of care and communicate effectively with families and the healthcare team.
• Respond to resident needs and emergencies promptly, ensuring a safe and supportive environment.
• Promote patient dignity, respect, and comfort through compassionate care.
• Adhere to all safety protocols, infection control guidelines, and regulatory requirements.
  
  
  

Qualifications:

• Valid LPN license in the state of Ohio.
• Compassionate and patient-centered approach to care.
• Strong clinical skills and experience in long-term care or rehabilitation settings preferred.
• Excellent communication and organizational skills.
• Ability to work both independently and as part of a team.
