Position Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team at Crown Heights Center for Nursing and Rehabilitation. As an LPN, you will play a critical role in providing high-quality care to our residents, supporting their medical and emotional needs, and ensuring their comfort and well-being. You will work closely with our healthcare team to promote health, healing, and rehabilitation.
Key Responsibilities
- Conduct rounds to observe residents and assess nursing care
- Review and implement Nursing Care Plans to address residents' physical, emotional, and rehabilitative needs
- Administer and transcribe physician orders accurately
- Collaborate with staff to deliver comprehensive, high-quality care
- Manage medication ordering, administration, and documentation
- Ensure accurate and thorough documentation of clinical records
- Participate in interdisciplinary meetings to improve care and resolve issues
- Provide direct patient care as needed, including monitoring vital signs and administering treatments
- Assist with identifying and addressing care improvement opportunities
- Perform additional duties within the LPN scope of practice as necessary
