What are the responsibilities and job description for the Case Manager - Full Time, Days position at Crozer Health?
As a Case Manager at Crozer health, you will utilize an interdisciplinary approach to the delivery of efficient and quality patient care by communicating and facilitating the patient's plan of care with an emphasis on the discharge plan, thereby decreasing fragmentation of hospital services and supporting quality care delivery.
The Case Manager uses nursing and case management knowledge to promote appropriate allocation of hospital and other resources, while striving to provide high quality care to each patient in a cost-effective and timely manner. Support services provided by the Case Manager include, but are not limited to, utilization review, case management, weekend and holiday case management, care transition, collaboration with physicians and social workers for care coordination and discharge planning.
Age range of patient population served: pediatric, adolescent, adult, geriatric.

Education and Training
- Bachelor’s degree in Nursing or related field.
- Current licensure to practice Professional Nursing in the Commonwealth of Pennsylvania
- CCM or equivalent certification preferred.
Experience
- Prefer previous acute care case management experience within the past 3 years. In lieu of prior case management experience, a minimum of 5 years of recent acute care or home care experience can be substituted.
Skills
- Good communication and interpersonal skills are essential for this role. Basic computer skills are also needed.
