Job Summary

The Remote Licensed Pediatric / OB Social Worker (LSW or LCSW) is responsible for coordinating social services for health plan members, working closely with the Pediatric OBGYN team. This position collaborates with healthcare stakeholders and members across the continuum of care, developing individualized plans that address psychosocial and financial factors impacting care. The role acts as a key resource for both members and the healthcare team, identifying and managing referrals to community resources to ensure optimal care.

You will be working within a collaborative Pediatric OBGYN team, which includes one other social worker and six RN case managers. The team is focused on providing high-quality, patient-centered care, and fostering an inclusive, supportive culture. The team is committed to delivering personalized care, emphasizing teamwork and effective communication.

Pay : $33.25 – 40.25 an hour based on experience (overtime will be paid at 1.5 times the normal hourly pay rate).

Hours : Full Time - Monday-Friday 8am-5pm

Job Duties :

  • Identify and initiate referrals for social service programs, including financial, psychosocial, and community or state services.
  • Manage the social service plan of care throughout the continuum of care as the single point of contact, including outpatient services.
  • Document all member assessments and services provided, including referrals to social service programs and follow-up to ensure completion.
  • Meet with members in their homes or at their healthcare provider's office when requested, deemed appropriate, or warranted.
  • Assist health plan members in inpatient settings and work with inpatient case management teams to facilitate safe transitions of care.
  • Participate in Interdisciplinary Team meetings and Utilization Management rounds as needed.
  • Perform other duties as assigned, including providing support during crises or transitions in care.
  • Requirements

  • Bachelor’s Degree in Social Work (BSW) required.
  • State of Nevada Social Work license required.
  • A minimum of 2 years of social work experience in acute care, clinic, home health, or case management settings.
  • A working knowledge of social work theory, practice, and diagnostic tools.
  • Familiarity with state and federal programs, community-based services, and applicable rules and regulations.
  • Knowledge of advanced directives, legal regulations, and reporting of abuse is required.
  • Ability to communicate effectively with diverse member populations and be adaptable to the needs of members and their families.
  • Ability to work independently with minimal supervision in varied settings, demonstrating initiative and problem-solving capabilities.
  • Proficient in using Microsoft Word and navigating within a Windows environment.
  • Highly Preferred
