About the Job
CSI Pharmacy is seeking a skilled Clinical Pharmacy Specialist to join our dynamic team. As a key member of our specialty pharmacy, you will be responsible for performing specified distributive and clinical pharmacy activities related to providing safe and effective home infusion therapies.
Key Responsibilities
- Develop and oversee policies and procedures establishing safe practices for infusion therapy handling in the pharmacy, during transport, at the suite, and patients' homes.
- Coordinate and facilitate new referrals/admissions through collaboration with nursing, customer service, and distribution teams.
- Review, receive/verify new patient orders, and document the same on the respective order form.
- Assist physicians and other healthcare professionals in planning and implementing patient care in the home to ensure safe and effective drug therapy.
- Conduct initial and ongoing assessment of specific patient, drug, and disease information to identify real and potential drug-related problems or needs.
Qualifications
- Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master of Science degree in Pharmacy desirable) from an accredited college of pharmacy.
- Currently licensed as a registered pharmacist in the state of employment.
- A minimum of 2 years in specialty pharmacy or ICU/critical care experience, plasma-derived therapy experience highly preferred.
