Job Summary
We are seeking a highly skilled and compassionate Pharmacist to join our dynamic team at CSI Pharmacy. The successful candidate will be based out of our Nash, TX location.
Key Responsibilities:
- Adhere to organizational policies and procedures, ensuring the delivery of safe and effective home infusion therapies.
- Develop and oversee policies and procedures establishing safe practices for infusion therapy handling in the pharmacy, during transport, at the suite, and patient's home.
- Coordinate and facilitate new referrals/admissions through collaboration with nursing, customer service, and distribution teams.
- Review, receive/verify new patient orders, and document the same on the respective order form, ensuring accurate and timely communication.
- Assist physicians and other healthcare professionals in planning and implementing patient care in the home, ensuring safe and effective drug therapy.
- Conduct initial and ongoing assessments of specific patient, drug, and disease information to identify real and potential drug-related problems or needs.
- Assist in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy.
- Monitor outcomes from drug therapy by collecting, evaluating, and interpreting laboratory data, clinical data, and other pertinent patient information.
- Communicate results of therapeutic drug monitoring activities to physicians, nursing staff, and other relevant personnel.
- Program infusion control devices as indicated.
- Provide consultation and answer inquiries from patients, families, physicians, and other healthcare professionals regarding drugs and their use.
- Maintain thorough and accurate documentation in patient/client records of all clinical activities and communication with patients, physicians, and other healthcare professionals.
- Participate in preparing progress reports for referring physicians on a regular basis.
- Process and check medications, supplies, and equipment prior to dispensing from the pharmacy.
- Maintain clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars, and conferences.
- Participate in on-call rotation, providing 24/7 support to patients and colleagues.
- Perform marketing support functions as requested.
- Participate in orientation/training programs for new personnel and current staff members.
- Participate in departmental QA activities, ensuring high-quality services.
- Attend designated nursing department meetings as scheduled.
- Maintain accurate and up-to-date pharmacy records in accordance with applicable laws and regulations.
- Responsible for handling customer complaints in accordance with Company policies and advising management promptly.
- Educate referral sources on CSI services relating to customer needs and benefits.
- Complete and submit all required reports in a timely manner.
- Maintain current files and other records in accordance with Company instructions and requirements.
- Collaborate with Senior Management to improve efficiency and product knowledge to meet Company goals.
- Assists management in special assignments, such as market tests and surveys.
- Provides high-quality services to home-care patients and referral sources.
- Attends and participates in conventions, trade shows, and in-services as deemed appropriate by management.
- Consistently represents the company in a professional manner.
- Maintains effective working relationships and cooperates with Company personnel.
- Adheres to the Company's compliance requirements as stated in the policy and procedure manual and all other related policies.
- Perform other duties and responsibilities as assigned.

Education and Experience
A Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master of Science degree in Pharmacy desirable) from an accredited college of pharmacy is required.
Currently licensed as a registered pharmacist in the state of employment is mandatory.
A minimum of 2 years in specialty pharmacy or ICU/critical care experience – plasma-derived therapy experience is highly preferred.
Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses, and other healthcare professionals.
