What are the responsibilities and job description for the Referral Intake Coordinator - Hospice Home Care position at CSS Medical Center at Windber?
This position is responsible for assuring that each and every referral inquiry received is assisted using all the organizations resources to access their Hospice benefit. Provides quality structure and processes to the referral process through consistent intake information and effective communication in order to provide continuous quality
care for patients. This position is also responsible for analyzing patient medical records and performing associated processes that have a direct impact on the timeliness of chart completion, coding and reimbursement. Performs administrative job duties to ensure efficient program operation and coordination of Hospice Home Care and Inpatient Unit. High School diploma or general education degree (GED) required. Associates degree or equivalent from a 2 year college or technical school in the area of secretarial, billing or administrative assistant studies preferred. Two years of experience in a healthcare setting is required. Home Health & Hospice experience a plus. Customer service/marketing experience strongly preferred.
