Job Description:
SUMMARY
The Housing Self Sufficiency Coordinator will provide CTCLUSI Tribal members with comprehensive community, and social services support to promote healthy and stable family units with a long-term goal of self-sufficiency. The Housing Self Sufficiency Coordinator will assist in securing safe and stable housing. Along with providing education to Tribal members in a wide variety of subjects, including but not limited to childcare, parenting, obtaining employment, and healthy lifestyles. The Housing Self Sufficiency Coordinator is responsible for working directly with housing clients and service providers to coordinate comprehensive service delivery that results in the enhancement of the social and economic well-being of the Tribe and Housing Department.
PRINCIPAL ACTIVITIES & RESPONSIBILITIES
* Receive referrals for at-risk families.
* Assessing clients’ housing needs and financial capabilities.
* Develop, implement and monitor individualized case plans for Tribal families. Such case plans must address the safety, health, education, housing, and emotional needs of any family or individuals.
* Assist Tribal members in accessing local and state services and programs.
* Provide client transportation as needed and approved.
* Participate in staff meetings and collaborative department meetings.
* Providing support for life skills education and connecting participants to community resources.
* Conduct home visits, assess individuals’ potential barriers, provide coaching, and support as needed.
* Provide referrals to improve housing retention, stability, quality of life and self-sufficiency.
* Maintain strict client confidentiality.
* Assist with all reporting for funding sources as well as required internal reporting requirements relating to current grants and funding.
* Maintain appearance standards as outlined in CTCLUSI policies.
* Must be able to interact with Tribal members and the general public in a courteous, professional, and efficient manner.
* Ability to communicate effectively, both verbally and in writing.
* Maintain a good attendance record.
* Other duties as directed by management.
* Coordinates with local service providers to ensure that program participants are linked to supportive services needed to achieve self-sufficiency, acts as a liaison between the residents and local service providers.
* Provides general case management which includes intake, assessment, education, and referral of residents to service providers in the general community.
* Establishes familiarity with community services; maintains updated information on health, educational, vocational, and social services; maintains a network of contact persons in community organizations and government benefits programs for potential referrals.
* Coordinates and sponsors events, which may include subjects relating to health care, job search seminars, life skills training, etc.
* Provides a variety of life-skills training to resolve financial and/or behavioral problems affecting tenant’s continued housing via direct training or referrals to community agencies. These trainings and/or referrals may include, but are not limited to, nutrition, transportation, conflict resolution, basic money management, problem-solving and other cognitive skills, financial literacy, credit repair, interpersonal skills, job training/search/placement, disability services counseling, meal services and/or assistance with activities of daily living for elderly/disabled residents.
* Collect and report on program participant demographic data that can be used in applying for grants or developing procedures and policies that better meet the needs of those served.

* Must be able to walk, talk, hear, use hands to handle, feel or operate objects, tools, or controls, and reach with hands and arms.
* Vision abilities required by this job include close vision and the ability to adjust focus.
* May be required to push, pull, lift, and/or carry up to 40 pounds.
WORKING CONDITIONS & ENVIRONMENT
* Moderate noise level with frequent interruptions and distractions.
* Ability and willingness to work a flexible schedule including the occasional evening and/or weekends.
* Ability and willingness to travel locally and state-wide as needed.
