Hospice Nurse
Hospice nurses provide constant and ongoing care for elderly and terminally ill patients by evaluating patient needs, creating care plans, and providing end-of-life support to patients and their families.

Job Summary:
- Acts as case manager making necessary adjustments in patient's plan of care related to symptom changes and changes in care needs.
- Provides end of life care and coordination at time of death and bereavement support following death
- Acts as case manager making necessary adjustments in patient's plan of care related to symptom changes and changes in care needs
- Reviews and plans patient care and or discharge during scheduled case conference meeting and documents accordingly.
- Acts as Liaison for patent and/ or caregiver with MD and community health care partners.
- Strives to lower cost of care through managing mileage, supplies and visit scheduling.
- Completes all documentation related to patient contact; assessments; SOC; Recerts.
- Provides on call coverage for both Home Care and Hospice as assigned
- Upholds and promotes patient safety and quality.
- Demonstrates and encourages team behavior and exceptional patient/guest experiences.
Education:
Associates degree in Nursing and currently licensed in the State of Alabama required.
Experience:
Minimal 2 years’ experience in a hospital setting working in Med/Surg; Critical Care or ER. Previous Home Care or Hospice experience preferred.
Additional Skills / Abilities:
Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize schedule to meet patient needs.
Pay: $24.00 - $40.00 per hour
Expected hours: 8 per week
