Position Overview:
As a Bi-Lingual Mental Health Advocate, you will work full-time (40 hours per week) to provide individualized clinical support services to consumers, offering consistent, face-to-face contact to ensure optimal psychosocial functioning. You will serve as a vital member of the PACT (Program of Assertive Community Treatment) Team, assisting with case management, medication delivery, and educating/supporting natural supports such as the individual's family.
What Does This Position Look Like:
The Bi-Lingual Mental Health Advocate serves as an active member of the PACT Team, providing case management and/or specialized services to consumers. The role includes participating in PACT medication deliveries and collaborating closely with consumers and team staff to assess individual needs, set treatment goals, develop action plans, and evaluate progress. The advocate monitors the effectiveness of services delivered, ensuring consumer satisfaction and quality of care. In addition, the position provides education and support to family members and other natural supports involved in the consumer's recovery. All services must be documented accurately, completely, and in a timely manner, following agency guidelines for record keeping and billing.
