Job Summary
Responsible for managing referrals, receive, process and manage incoming referrals from agencies, healthcare provider and community organization on prospective clients as well as needed for client?s files under the direct supervision of a licensed clinician.
Essential Functions:
- Comply and abide with the agency?s Code of Ethics.
- Comply with the agency?s policies and procedures, as well as applicable laws, regulations and governing alcohol and other drug abuse services particularly the Federal confidentiality regulations.
- Verify eligibility and appropriateness of clients for the program.
- Build and Maintain Relationships: Serve as the primary point of contact for referring agencies. Develop and maintain strong working relationships with referral sources to ensure consistent communication and collaboration.
- Client Intake Coordination: Work with clients referred by agencies to gather required information and ensure a smooth admissions process. Coordinate with internal teams to schedule assessments and admission dates.
- Documentation and Reporting: Maintain accurate records of referrals, communication logs, and follow-ups in the system. Generate reports to track referral trends and provide feedback to management.
- Outreach and Education: Provide training and education to referral sources about the inpatient program, services, and admission criteria.
- Quality Assurance: Ensure timely follow-up with referral sources regarding client outcomes or status. Address concerns or issues from referral agencies to maintain positive partnerships.
- Collaborate with Internal Teams: Work closely with clinical staff, admissions teams, and case managers to ensure a seamless transition for clients. Share feedback from referring agencies to improve program quality and accessibility.
- Compliance: Ensure all activities align with HIPAA regulations and other confidentiality standards. Adhere to program policies and state/federal guidelines.
This role is crucial in bridging the gap between external agencies and the inpatient program, ensuring smooth communication and client transitions.
Duties and Responsibilities:
Assist in the following tasks, as delegated, under direct supervision-
- Familiarity with company programs e.g. NJSAMS, DAISY, ASI, Windows 2000 Pro/Win98 SE; MS Office, MS Publisher, Adobe Reader, Internet access
- Knowledge of word processing, spreadsheets, and database management
- Manage phone, e-mail correspondence and inquiries; provide customer service support via telephone or in person
- Maintain office equipment; order supplies as needed/approved
- Able to perform your daily task in a timely and professional manner, making all deadlines
- Perform additional duties that are assigned
- Filing, answering phones and taking messages
- Other tasks as delegated
Ability to:
- Speak English (Spanish languages are a plus), as well as write clearly and coherently in English.
- Work cooperatively with others.
