- Review medical records and documentation to ensure accuracy and completeness
- Collaborate with healthcare professionals to develop care plans for patients
- Coordinate and schedule home care services for patients in need
- Arrange transfers and admissions to nursing homes or other healthcare facilities
- Ensure compliance with Medicare guidelines and regulations
- Maintain confidentiality of patient information and adhere to HIPAA regulations

- Strong knowledge of medical terminology and physiology
- Experience in nurse management or related healthcare roles
- Excellent organizational and time management skills
- Attention to detail and ability to review documentation accurately
