What are the responsibilities and job description for the MDS Coordinator - RN position at Cura of Monticello?
Duties:
- Coordinate and oversee the MDS (Minimum Data Set) process for patients in a healthcare setting
- Ensure accurate and timely completion of MDS assessments and documentation
- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments
- Review medical records and interview patients, families, and caregivers to gather relevant information
- Analyze data and identify potential issues or discrepancies in MDS assessments
- Develop care plans based on MDS assessments and collaborate with healthcare professionals to implement appropriate interventions
- Monitor and evaluate patient progress and update care plans as needed
- Provide education and training to staff on MDS documentation requirements and processes
Skills:
- Proficiency in using PCC or similar electronic health record (EHR) systems
-Proficiency in SNF MDS chart for TCU and LTC
