Job Title: Nurse Practitioner or Physician Assistant - Per Diem
CuraNa Health is a provider of value-based primary care services for the senior living industry. Our clinicians serve more than 1,500 senior living community partners across 34 states.
We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our healthcare team, delivering high-quality primary care in senior living facilities.
Key Responsibilities:
- Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents, focusing on preventive care and chronic disease management.
- Conduct regular rounds to assess residents' conditions, adjust treatment plans, and address any emergent health concerns.
- Collaborate with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to the residents' needs.
- Educate residents, families, and facility staff on disease prevention, wellness, and managing chronic illnesses to support better health outcomes.
- Maintain accurate, timely, and thorough documentation within the electronic medical record (EMR) system, ensuring compliance with regulatory standards and organizational protocols.
- Participate in quality improvement initiatives to enhance care delivery, reduce hospital readmissions, and optimize patient satisfaction within the facility.
Requirements:
- Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
- Masters Degree in Nursing or Physician Assistant Studies
