- Provide traditional primary care for older adults, including diabetes, hypertension, hyperlipidemia, mild cognitive impairment, CHF, and a history of coronary artery disease ">
- No procedures performed ">
Schedule
">Monday-Friday, 8 AM - 5 PM, with flexibility for a 4-day workweek
">Credentialing Timeframe
">Approximately 30 days
