Job Responsibilities:
- Assess the psychosocial status of patients in relation to their illness and environment and share findings with the care team.
- Conduct social evaluations and create intervention plans based on findings.
- Maintain accurate clinical records for patients referred to social work.
- Prepare detailed clinical and progress notes.
- Provide information and referral services to patients and their families/caregivers, addressing practical and environmental needs.
- Approach every task with a "passion for compassion."
What We’re Looking For:
- Master’s degree in Social Work from a program accredited by the Council on Social Work Education.
- State of Michigan licensure.
- At least one year of experience in health care.
- A genuine, compassionate heart for caring for others.
