Schedule: Monday - Friday, 8:30 AM - 5:00 PM

Essential Job Functions/Responsibilities:
- Manage patient scheduling for clinical teams, ensuring proper care coordination and follow-up.
- Answer phones and provide high-level customer service to patients, families, and healthcare providers.
- Handle incoming referrals, scheduling registration visits, and coordinating admission assessments.
- Maintain accurate records, logs, and patient information in the EMR system.
- Assist in verifying patient insurance information and ensuring complete referral documentation.
- Participate in weekend and after-hours on-call rotations to cover phone lines and emergency scheduling.
- Provide clerical support to the referral management and intake department, including maintaining schedules and tracking patient progress.
- Proactively collaborate with the referral team, clinical staff, and external healthcare providers to ensure smooth transitions for patients.
Position Qualifications:
- High school diploma required; college degree preferred.
- Proficiency in Microsoft Office.
- Strong organizational, multitasking, and time management skills.
- Excellent customer service and telephone communication abilities.
- Flexible, patient, and cooperative, with a strong work ethic.
- Experience in healthcare or referral management is a plus.
