Are you a detail-oriented professional who excels at multitasking and ensuring smooth operations? At Custom Home Health, we're seeking a dedicated Team Coordinator to manage administrative tasks, coordinate care, and play a vital role within our referral management and intake department. You'll ensure efficient scheduling, patient communication, and contribute to the overall excellence in patient care.

Schedule: Monday - Friday, 8:30 AM - 5:00 PM

Essential Job Functions/Responsibilities:

  • Manage patient scheduling for clinical teams, ensuring proper care coordination and follow-up.
  • Answer phones and provide high-level customer service to patients, families, and healthcare providers.
  • Handle incoming referrals, scheduling registration visits, and coordinating admission assessments.
  • Maintain accurate records, logs, and patient information in the EMR system.
  • Assist in verifying patient insurance information and ensuring complete referral documentation.
  • Participate in weekend and after-hours on-call rotations to cover phone lines and emergency scheduling.
  • Provide clerical support to the referral management and intake department, including maintaining schedules and tracking patient progress.
  • Proactively collaborate with the referral team, clinical staff, and external healthcare providers to ensure smooth transitions for patients.

Position Qualifications:

  • High school diploma required; college degree preferred.
  • Proficiency in Microsoft Office.
  • Strong organizational, multitasking, and time management skills.
  • Excellent customer service and telephone communication abilities.
  • Flexible, patient, and cooperative, with a strong work ethic.
  • Experience in healthcare or referral management is a plus.
