Position Overview
- As a Primary Care Physician, you will play a vital role in delivering comprehensive care to seniors, allowing them to remain in their homes and communities. Our holistic approach focuses on treating the whole person rather than just symptoms. You will work within a collaborative interdisciplinary team (IDT) to assess, plan, and oversee participant care, ensuring alignment with our values and mission.
- Key Responsibilities:
- Conduct in-person comprehensive assessments and collaborate with the IDT to create individualised care plans.
- Oversee participant care in nursing homes and hospitals, including managing admissions, discharges, and coordinating with medical specialists.
- Provide guidance and support to Nurse Practitioners and other IDT members.
- Engage in discharge planning with community-based physicians and the IDT.
- Actively participate in IDT meetings, communicate changes in participant status, and coordinate 24-hour care delivery.
- Regularly visit nursing homes as required by participant needs.
- Participate in rotating night and weekend on-call duties.
- Completion of an accredited residency program (Accreditation Council for Graduate Medical Education).
- Master’s or Doctorate Degree in Medicine.
- Board Certification in Internal Medicine or Family Practice is required; Geriatrics certification preferred.
- Active, unrestricted California Physician License.
- State CDS Certification and DEA Registration.
- Valid California Driver’s License with a clean DMV record.
- Recent Vehicle Insurance documentation.
- 1 to 2 years of experience in a similar role.
- Associate level seniority.
- No management experience required.
- Doctorate Degree is the minimum educational requirement.
- Willingness to travel frequently.
