Job Summary
The Assessment Specialist schedules and obtains all required documentation on youth ordered to Alternative Case Plan Committees (ACP). The Assessment Specialist analyzes documents and initiates the placement referrals to the appropriate facilities. The Assessment Specialist coordinates with all placement facilities to ensure a smooth transition to the facility before and after a youth is ordered. The Assessment Specialist assists with gathering medical and mental health records from outside agencies to support referrals for Diagnostic Clinic evaluations.
Essential Duties and Responsibilities

Using a strengths-based, culturally competent approach, interviews youth and parent/guardian and reviews case documents to complete assessments and/or screens.
Evaluates, analyzes, and interprets psychological/psychiatric, medical, chemical dependency assessments, CCDCFS reports, victim impact statements, sex offender, and other relevant reports. Presents results to Court and case planning team meetings. Develops placement plan to be presented to facilities at time of referral.
Reviews and analyzes all required residential placement material and is also responsible for scheduling Alternative Case Plan Team meetings. Participates as an active member of the Alternative Case Plan Team.
Coordinates the pre-admission process for adjudicated youth who are court ordered into the residential treatment and/or Community Correctional Facility.
Initiates referral to appropriate residential facility in a timely manner and actively participates in pre-admission processing including pre-placement interviews and transportation of child. Establishes open and timely communication with placement facilities regarding acceptance and rejections of referrals.
Completes referrals, arranges interviews, and elicits decisions from placement agencies in a timely manner. Responsible for completing Medicaid and Title IV-E maintenance cost reimbursement material for youth prior to placement outside their home in private residential treatment.
Assembles and completes required documents related to Medicaid and Title IV-E eligibility applications.
Assists with records coordination tasks by gathering reports and documents requested by the Diagnostic Clinic and Probation Department for assessments and screenings. Obtains signatures on appropriate Release of Information documents with youth and families. Follows up on requests with outside agencies and shares documents with appropriate parties in the Diagnostic Clinic and Probation Department.
Contacts pertinent Court and/or collateral agency professional staff for case clarification as necessary for final decision-making.
Gathers required Title IV-E eligibility documents for review and input into SACWIS by the SACWIS Coordinator per policy standards.
Prepares required statistical reports and other written correspondence as necessary.
Confers with manager to evaluate and participate in case priorities and case consultations, including initial and final placement and aftercare plans, progress and lack thereof, to formulate recommendations regarding the duration/ status of services.
Performs other duties as assigned.
Supervisory Responsibilities
Has no direct supervisory responsibility for staff members.
Equipment Operated
Standard office equipment and office technology such as copy machines, personal computers and telephones.
Confidential Data
All court-related information regarding hearings, etc. that the employee may come in contact with by working in the facility.
Working Conditions
Potential exposure to hazardous situations that occur in the Court environment. Must be able to tolerate exposure to evidence and testimony that may be disturbing, clients who may potentially be verbally or physically uncooperative, allergens, such as perfumes and dust.
Usual Physical Demands
While performing the duties of this job, the employee is required to sit, to speak clearly, and to hear. The employee must occasionally lift and/or move up to 10 pounds.

Must be able to effectively manage time, set priorities and work under deadline. Skills in interviewing, and assessment. Must have demonstrated ability to work as a team member with multidisciplinary teams that cross systems. Demonstrated knowledge of adolescent growth and development, gender specific needs, human behavior, and family dynamics. Ability to work and plan with families using a strengths-based approach. Effective oral and written communication skills. Must be computer literate. Ability to make use of referrals; maintain confidentiality; establish and maintain positive working relationships; make use of supervision. Must be culturally competent. The ability to effectively manage stress; excellent interpersonal skills. Flexibility in work hours to accommodate program needs including, but not limited to, case consultation on an emergency or as needed basis 7 days/week, 24 hours/day, in person or by telephone.
Education and/or Experience
Bachelor’s Degree in Behavioral and/or Social Sciences such as a degree from an accredited school of social work, psychology, sociology, corrections or related fields.
AND
Three (3) years of work experience with adolescents or relevant field experience in case management or other collateral systems interfacing with the juvenile justice system.
Licensure or Certification Requirements
Must possess a valid Ohio driver’s license and maintain valid automobile insurance, with a clean driving record and ready access to a safe functioning automobile.
