Enjoy the flexibility to choose your hours and workdays, ranging from 0.2 to 0.8 FTE (8 to 32 hours / week), with flexible shifts available. Responsibilities : Provide direct, compassionate patient care in home settings using the nursing process. Collaborate with patients to identify barriers and create personalized care plans. Prevent hospital readmissions through proactive care and education. Complete OASIS assessments and medication reconciliations in an electronic medical record system. Qualifications Required : Current Minnesota RN#license. Ability to obtain#American Heart Association BLS certification#within 30 days of hire. Valid Minnesota driver#s license. Preferred : Previous experience in Home Care and / or Hospice settings. Key Skills : Compassionate and patient-focused care. Strong communication and interpersonal abilities. Confidence in teaching and guiding patients on disease processes and care plans.
