Job Description :
- Provide physical therapy services within assigned territories, primarily in Area #8 (Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington) and Area 2 (Oakland, Berkeley, Albany, Emeryville, Piedmont).
- Additional travel outside these areas may be required.
- Weekend coverage may be required.
- Must be willing to travel outside the designated area or work weekends if necessary to maintain weekly visit expectations.
- Responsible for providing care to Covid and Covid PUI patients.
- Perform routine visits, evaluations, discharges, and various types of follow-ups, understanding how each visit converts to points and corresponding hours.
- Documentation, travel, and communication times are accounted for within the all-inclusive rate.
- Ensure all issued credentials and health screenings are up to date and compliant.
- Maintain adherence to contract specifications, including the timely submission of requested time off.
- Ensure flexibility and commitment to cover holiday requirements.
