Job Summary :
As a member of a multidisciplinary care management team, the Nursing Case Manager is accountable and responsible for coordinating the care and services of selected patient populations to ensure high-quality care, efficient resource utilization, and optimal outcomes.
Essential Responsibilities :
Comprehensively assesses patient goals, including biophysical, psychosocial, environmental, discharge planning needs, and financial status.
Closely monitors patients’ progress and evaluates changes in medical needs that could affect current and future medical treatment.
Procures necessary services and advocates on behalf of the patient and family, particularly in relation to scarce resources.
Collaborates with third-party payers to resolve issues affecting the continuity of patient care.
Coordinates and directs the discharge planning process with patients, family members, the healthcare team, and community services.
Reviews admission data within the designated timeframe to assess the suitability of the level of care according to ISD-A criteria and / or established guidelines.
Required Skills & Experience :
Minimum of three (3) years of clinical nursing experience in an acute care facility.
Strong knowledge of nursing practices and procedures.
Exceptional clinical assessment skills.
Knowledge of community resources available to patients, families, and healthcare teams that facilitate recovery.
Familiarity with third-party payer issues, including reimbursement processes and regulatory guidelines.
Ability to explain complex medical and technical information in an easily understandable manner to patients and families.
Strong interpersonal skills to gather necessary information for assessing patient needs and determining appropriate care.
Preferred Skills & Experience :

Required Education :
Graduate from an accredited school of nursing.
Preferred Education :
Bachelor of Science (or higher) in Nursing from a Board-approved program.
Required Certifications & Licensure :
Current RN License issued by the Oklahoma State Board of Nursing, or a current multistate Compact RN License (eNLC).
Basic Life Support (BLS) certification issued by the American Heart Association required upon hire.
CWS Health Job ID #20. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
