Key Responsibilities
- Provide proactive social services to support the biopsychosocial needs of patients admitted to the hospital
- Collaborate closely with the inpatient care team to ensure continuity of care
- Develop and implement effective discharge plans, identifying and addressing barriers to discharge
- Facilitate patient/family care conferences and assist with Advance Directives and Power of Attorney decisions
- Connect patients with community resources to support recovery and well-being
- Provide supportive counseling, coaching, and linkage with essential services
- Maintain accurate records and participate in multidisciplinary team meetings
Requirements
- Bachelor's or Master's degree in Social Work (BSW or MSW)
- Valid State of Alaska Driver's License or ability to obtain within 6 months
- Certifications: BSW or MSW in social services required
- Strong understanding of ethical practice, case management principles, and knowledge of community support resources
- Excellent problem-solving and conflict resolution skills
- Ability to maintain confidentiality and comply with regulations and policies
