About the Job
Cynet Health is seeking an experienced Social Worker to join our team in Newark, DE. In this role, you will work collaboratively with RN Case Managers and Utilization Managers to resolve psycho-social barriers and support discharge needs for our patients. Responsibilities
This role involves:
- Providing education and support to patients and their families to ensure a smooth transition from hospital to home or other care settings;
- Collaborating with healthcare teams to develop effective care plans and coordinate services;
- Maintaining accurate records and reports, adhering to regulatory requirements and professional standards;
- Participating in quality improvement initiatives to enhance patient outcomes and satisfaction;
- Engaging in ongoing professional development to stay current with best practices and industry developments.
Requirements
To succeed in this role, you will need to possess a Master's degree in Social Work (MSW) or a related field, and be licensed as a Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW). A minimum of 2 years of experience in acute care social work is required, with a focus on working with neonates, children, and families. Certification as a Certified Case Manager (CCM) within 18 months of eligibility is also desirable. Strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment, are essential.
