- Job Title: RN Case Manager – Discharge Planning
Location: Jackson, MS 39216
- Shift Length: 8 Hours
Guaranteed Hours: 40 Hours per Week
- Shift Timing: 8:00 AM to 4:30 PM
Key Responsibilities:
- Manage a caseload of 15-20 patients per day for discharge planning.
- Collaborate with multidisciplinary teams to ensure smooth discharges.
- Ensure compliance with discharge criteria and patient care protocols.
Requirements:
- 2 years of acute care management and discharge planning experience.
- BLS Certification required.
- Active RN license.
Additional Info:
- No weekend requirements, but holiday rotation may apply.
- Orientation provided for 2 weeks.
