Key Responsibilities:

Assess patients' biopsychosocial needs, working closely with the inpatient multidisciplinary care team.
Manage discharge planning and collaborate with Inpatient Care Coordinators to ensure seamless patient transitions.
Perform swing bed assessments upon admission into subacute rehabilitation services.
Facilitate or co-facilitate patient/family care conferences to ensure understanding of care plans.
Assist patients in making healthcare decisions and guide them in navigating the healthcare system.
Identify community resources to assist patients and family/caregivers during hospitalization and recovery.
Support patients with financial or legal matters.
Assist with long-term care or assisted living needs.
Help patients with dental, podiatric, eye care, and hearing services.
Refer patients needing chemical dependence or mental health support to appropriate clinicians.
Documentation and Communication
Maintain detailed records in accordance with policy and regulations.
Document patient notes clearly and timely for healthcare providers.
Maintain patient care information confidentiality and comply with documentation standards.
Exhibit professional communication, conflict resolution skills, and multi-tasking abilities.
Collaboration and Teamwork
