Experience

3-5 years of case management experience in an acute inpatient setting

License

Registered Nurse (RN) with a valid, unrestricted State license or Licensed Social Worker (LMSW) with a valid, unrestricted State license

Certifications

Basic Life Support (BLS) certification from the American Heart Association

Must-Have

  • Experience with Epic EMR
  • Strong computer skills
  • Excellent verbal communication and written documentation skills

Job Description

  • Conduct comprehensive assessments of patients and their family/caregivers to determine biomedical, psychological, social, and functional needs.
  • Develop personalized, patient-centered care plans to optimize the patient’s care experience.
  • Engage patients and families as part of the care team through advocacy, communication, health education, resource identification, and service facilitation.
  • Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to help patients overcome barriers to goal achievement.
  • Provide counseling and interventions related to treatment decisions and end-of-life issues, including Advanced Care Planning.
  • Ensure seamless and safe transitions for patients between care settings.
  • Advocate for the appropriate delivery of services within the patient's health plan benefit structure.
  • Collaborate with members of the patient's treatment/care team to manage patients with complex medical and social needs and facilitate collaborative care conferences.
  • Continually evaluate the patient’s response to the care/treatment plan and make modifications when necessary.
  • Plan and participate in process improvement activities to reduce risk, including data collection, analysis, and follow-up interventions.
  • Facilitate interventions in cases involving child abuse and neglect, domestic violence, elder abuse, institutional abuse, and sexual assault.
  • Support department-based goals contributing to the success of the organization.
  • Perform other duties as assigned.
