- Strong computer skills
- Excellent verbal communication and written documentation skills
- Conduct comprehensive assessments of patients and their family/caregivers to determine biomedical, psychological, social, and functional needs.
- Develop personalized, patient-centered care plans to optimize the patient’s care experience.
- Engage patients and families as part of the care team through advocacy, communication, health education, resource identification, and service facilitation.
- Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to help patients overcome barriers to goal achievement.
- Provide counseling and interventions related to treatment decisions and end-of-life issues, including Advanced Care Planning.
- Ensure seamless and safe transitions for patients between care settings.
- Advocate for the appropriate delivery of services within the patient's health plan benefit structure.
- Collaborate with members of the patient's treatment/care team to manage patients with complex medical and social needs and facilitate collaborative care conferences.
- Continually evaluate the patient’s response to the care/treatment plan and make modifications when necessary.
- Plan and participate in process improvement activities to reduce risk, including data collection, analysis, and follow-up interventions.
- Facilitate interventions in cases involving child abuse and neglect, domestic violence, elder abuse, institutional abuse, and sexual assault.
- Support department-based goals contributing to the success of the organization.
- Perform other duties as assigned.
