Job Description

• Provide skilled home health care to patients with a variety of medical diagnoses including high-risk conditions such as heart failure, COPD, myocardial infarction, pneumonia, and stroke
• Perform post-operative care, including cardiopulmonary assessment, basic wound care, and advanced wound treatments such as ABI compression dressing and wound vac dressing
• Manage pleural, aseptic drain care, foley catheter change and care, staple/suture removal, and the use of glucometer and pro-time machine
• Administer SQ/IM injections, and perform lab draw venipuncture and PICC line or midline care and dressing change
• Conduct SQ port access and de-access procedures
• Complete home skilled visits ranging from six to eight per day
• Utilize charting systems efficiently, with Cerner charting experience preferred
• Comply with all safety and infection control policies, including maintaining a smoke-free environment
• Fulfill 20 hours of non-billable orientation and complete required modules pre-start and annually
• Self-schedule based on the needs of the HomeCare office, with shift hours varying from Monday to Friday, 8 am to 5 pm or 9 am to 6 pm, and may include Thursday to Monday or Friday to Tuesday shifts, with alternating weekend shifts
• Maintain accurate documentation using specified timesheet documentation system
  
  

Note: Candidates must meet compliance and pass/fail EKG exam within allotted attempts. Tasks may require floating within hospitals or affiliate hospitals nearby. Compliance documents must be completed by the specified deadline.
