- Provide, coordinate, and direct home nursing care following physicians' orders based on agency policies and procedures.
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
- Perform home visits and attend periodic office meetings.
- Drive to patient homes daily and to the clinic for weekly meetings, with travel up to 75 miles from the home clinic.
- Handle an average of 5-10 patients per day with productivity expectations of 32 points per week.
- Conduct various types of cases, including Start of Care, Wound Care, and Follow-up Visits.
- Complete all documentation within 24 hours of the patient appointment to ensure effective billing and patient care follow-up.
- Be open to on-call rotations and capable of adjusting to weekend requirements per agency needs.
- Maintain reliable transportation, a current driver's license, and agency-required liability insurance.
- Adhere to the required dress code by wearing scrubs and ensure proficiency with the electronic medical record system.
- Participate in occasional floating to nearby clinics to pick up patients if patient census is low.
