- Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings; must have reliable transportation, a current driver's license, and required liability insurance.
- Manage a caseload of 5-10 patients per day, equating to roughly 6-8 productivity points.
- Handle various types of cases, including Start of Care, Wound Care, and Follow-Up Visits.
- Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed at the standard rate.
- Documentation must be completed within 24 hours of the appointment with the patient.
- No guaranteed hours; expected to hit weekly productivity points of 30, equating to 40 hours per week.
- Familiarity with HomeCare HomeBase EMR is required; retraining is available if the system hasn't been used in the last 6 months.
- No on-call or weekend rotation is required.
- Expectation to accept every patient on the schedule, regardless of the distance from the previous appointment.
