- Certifications:** Basic Life Support (BLS) required
- Home Health & Hospice experience

- Familiarity with EMR system HomeCare HomeBase
- Conduct initial assessments of clients and their families to determine home health needs, including physical assessments and history of current and previous illnesses.
- Regularly re-evaluate client nursing needs and use health assessment data to formulate nursing diagnoses.
- Initiate and revise plans of care as client status and needs change; establish goals and incorporate therapeutic, preventive, and rehabilitative nursing actions.
- Include the client and family in the planning process.
- Administer medications and treatments as prescribed by the physician.
- Provide healthcare instructions to clients as appropriate based on assessment and plans of care.
- Identify discharge planning needs and implement plans prior to client discharge.
- Act as Case Manager when assigned, coordinating client care for assigned caseloads.
- Prepare clinical notes and update the primary physician when necessary and at least every sixty days.
- Communicate with physicians regarding client needs, report changes in client condition, and obtain physician orders as required.
- Ensure arrangements for equipment and other necessary items and services.
- Instruct, supervise, and evaluate home health aide care provided to clients.
- Provide care to patients within a 60-mile radius, including homes and nursing homes.
