Job Description
- Provide skilled nursing care and case management for assigned patients in their homes.
- Admit patients onto services, coordinate and integrate services to ensure continuity of care.
- Facilitate the achievement of quality, clinical, and cost outcomes; negotiate, procure, and coordinate services and resources needed by the patient or family.
- Create opportunities and systems to enhance outcomes; use the nursing process of assessment, planning, and implementation of care, including patient/family instruction and evaluation through an interdisciplinary team approach.
- May start as a Visiting RN and move to Case Manager.
- Provide skilled nursing and coordinate care for patients and family members through a team approach based on systematic evaluation as authorized by a physician.
- Observe, assess, evaluate, and plan interventions related to patients' physical and psychosocial statuses, including changes in condition and reactions to treatments or medications.
- Oversee care provided by Home Health Aides and CNAs and evaluate compliance with the plan of care.
- Ensure continuity of care through effective communication, information exchange, and case conferences with other disciplines and community health, support, and social agencies.
- Provide quality care and appropriate symptom management for positive clinical outcomes and promote optimal health and quality of life through teaching, counseling, interventions, and referrals.
- Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements as well as knowledge of current reimbursement rules and regulations.
- Participate in team scheduling coordination and on-call rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and making home visits as required.
- Participate in quality improvement activities and attend in-service training as assigned.
- Perform other duties as assigned by a supervisor.
