Shift: Monday to Friday, 8 AM to 5 PM with on-call responsibilities

Guaranteed Hours: 40 hours per week

Experience: Minimum 2 years of recent acute care, home care, or hospice experience

License: Current RN license

Certifications: BSN preferred

Must-Have: Valid driver's license, appropriate auto insurance, and reliable transportation

Job Description

• Provide skilled nursing care and case management for assigned patients in their homes.
• Admit patients onto services, coordinate and integrate services to ensure continuity of care.
• Facilitate the achievement of quality, clinical, and cost outcomes; negotiate, procure, and coordinate services and resources needed by the patient or family.
• Create opportunities and systems to enhance outcomes; use the nursing process of assessment, planning, and implementation of care, including patient/family instruction and evaluation through an interdisciplinary team approach.
• May start as a Visiting RN and move to Case Manager.
• Provide skilled nursing and coordinate care for patients and family members through a team approach based on systematic evaluation as authorized by a physician.
• Observe, assess, evaluate, and plan interventions related to patients' physical and psychosocial statuses, including changes in condition and reactions to treatments or medications.
• Oversee care provided by Home Health Aides and CNAs and evaluate compliance with the plan of care.
• Ensure continuity of care through effective communication, information exchange, and case conferences with other disciplines and community health, support, and social agencies.
• Provide quality care and appropriate symptom management for positive clinical outcomes and promote optimal health and quality of life through teaching, counseling, interventions, and referrals.
• Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements as well as knowledge of current reimbursement rules and regulations.
• Participate in team scheduling coordination and on-call rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and making home visits as required.
• Participate in quality improvement activities and attend in-service training as assigned.
• Perform other duties as assigned by a supervisor.
