Job Description
- Provide home health care to patients in a designated geographic area.
- Manage a patient ratio of 1:1, with up to 6 patient visits per day.
- Complete 30 productivity points standard.
- Use EPIC for charting.
- No weekend work or on-call requirements.
- Not required to float between like units.
- Mileage reimbursed in line with IRS standards.
- Flexibility within a 50-75 mile radius; locals accepted within this range at the same rate.
- Complete orientation of 40 non-billable hours.
- Modules required pre-start and annually, averaging 16 hours.
- Provide two supervisory references covering 1 year within the last three years in a similar specialty or setting.
- Ensure the capability to meet the job's point expectation.
