Job Description

• Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Travel to home visits and periodic office meetings; must be willing to travel up to 75 miles from the assigned clinic.
• Manage a caseload that averages 5-8 patients per day, with productivity expectations of 30 points per week.
• Types of cases include Start of Care, Wound Care, and Follow-up Visits.
• Be available for weekend and on-call rotations as required.
• Complete all patient documentation within 24 hours of the appointment.
• Attend weekly meetings at the clinic and float to nearby clinics if necessary to meet financial needs.
• Ensure proficiency in HomeCare HomeBase and OASIS systems within the last 6 months or complete a retraining course during compliance.
• Dress code requires scrubs.
