Job Description

• Perform the nursing process, including assessment, diagnosis, planning, implementation, and evaluation, using evidence-based practice.
• Coordinate patient care through effective partnerships, ensuring individualized, relationship-based patient care.
• Provide patient-centered care that reflects an understanding of cultural diversity and population-specific characteristics.
• Partner with patients and significant others using appropriate methods for communication and teaching.
• Coordinate the delivery and documentation of safe, quality patient care in an inclusive interdisciplinary environment.
• Demonstrate fiscal responsibility and promote the recruitment and retention of healthcare team members.
• Participate in and support performance improvement initiatives and promote shared governance.
• Complete all professional competency assessments and required education.
• Serve as a preceptor or mentor for other nurses and staff, modeling professional practice.
• Promote evidence-based practice, nursing research, and performance improvement.
• Uphold all policies, procedures, and clinical competencies, including legal, regulatory, and accreditation standards.
• Participate in data collection and use appropriate resources to address practice questions.
• Proactively plan for patient care and participate in protocol development for best practice outcomes.
• Support efficient use of human and material resources and provide leadership in problem-solving within the call center.
• Establish effective working relationships with physicians, patients, and families, educating them on procedures, medications, and wellness.
• Maintain strict confidentiality and ensure appointment preferences for emergency situations.
• Maintain and review patient records and other pertinent information.
