  • Job Description :
  • Ensure all prescreening documents are completed and submitted with the application.
  • Attach a current resume and proof of license verification for consideration.
  • Candidates may be declined if required documents are not attached.
  • Conduct therapy sessions focusing on speech rehabilitation.
  • Provide treatment plans tailored to individual patient needs.
  • Maintain documentation of patient progress and treatment plans.
  • Collaborate with a multidisciplinary team for comprehensive care.
  • Ensure compliance with all state and federal regulations regarding patient care.
  • Conduct comprehensive patient assessments and screenings.
  • Review the credentialing checklist, which includes :
  • Resume and three references
  • County criminal check for the past seven years
  • Federal criminal check for the past seven years or more
  • Multi-state criminal search for the past seven years or more
  • Nationwide Sex Offender Search for the past seven years or more
  • HHS / OIG Search and GSA Search
  • Social Security Number Trace
  • 2 Step PPD or Quantiferon or proof of positive PPD and chest X-ray
  • SAM Search
  • Bilingual capability and holiday coverage are not required for this position.
